UPDATE: As of June 2021, Date of Birth is no longer a mandatory field (as is mentioned in this video). Do keep in mind that by turning off this field, you can not enforce the Starting Adult Age feature of WaiverSign so it's recommended to avoid turning off DOB unless you've consulted legal counsel.
